Passport Services at Warner Library are POSTPONED until further notice

Do you need a new U.S. Passport? Is your existing passport more than 15 years old? Are you changing from a passport for a Minor (under age 16) to a passport for an Adult (age 16 and over)? If so, come to the library to process your new passport application.

Forms should be completed before appointments, but not signed.

Please note that renewals can only be processed through the U.S. Postal Service.

Requirements

• Proof of U.S. Citizenship: Original or Original Certified Copy of Long Form Birth Certificate or Original Certification of Naturalization.
• Photo ID: Driver’s License, Passport, or Military ID, and a photocopy of the front and back of it.
• One recent color photograph. Nearby stores to purchase passport photos include:  CVS, Costco, Walgreens
• For Minor Applicants: Both parents must be present and sign application; or one parent appears, signs, and submits second parent’s notarized Statement of Consent, along with valid ID; or one parent appears and submits Primary Evidence of Sole Custody to apply.

Note: Each applicant must appear in person.

Fees & Processing Times

Fees and processing times vary with choice of service.

For Routine Service, processing can take between 6 to 8 weeks. For payment, please bring:

• $35 in cash or a check payable to the “Warner Library” for Execution Fee.
• One (1) check per application payable to the “U.S. Department of State” for Application Fee(s) in the amounts as follows:
  • Adult Passport Application – $110
  • Minor Passport Application – $80

For Expedited Service, processing can take between 2 to 3 weeks. For payment, please bring:

• $35 in cash or a check payable to the “Warner Library” for Execution Fee.
• One (1) check per application payable to the “U.S. Department of State” for Application Fee(s) in the amounts as follows:
  • Adult Passport Application – $170
  • Minor Passport Application – $140

No cash or debit/credit card payments accepted for Application Fees.

Passport Card

For an additional fee, an applicant can purchase a passport card valid for travel by land and by sea only to the following destinations:

• Canada
• Mexico
• The Caribbean
• Bermuda

Adult card fee:  $30
Under age 16 card fee:  $15
